Is there a difference in the outcome of primary total knee arthroplasty performed using kinematic versus mechanical alignment?

Abstract
Kinematic alignment (KA) shows improved patient-reported outcome measurements (PROMs) in early recovery but equivalence past mid-term follow-up. KA targets a native joint line obliquity and reapproximates pre-arthritic alignment, thus less soft-tissue adjustment may account for early differences. Several randomized controlled trials (RCTs) and systematic reviews fail to identify a durable clinically meaningful difference; Future non-commercial, non-biased, level-one studies are needed to elucidate long-term clinical outcomes and cost-effectiveness.
